What the quiz collects, and why.
The quiz asks about your health, so this notice sets out what we collect, why we want it, and who sees it, before you decide whether to keep your answers.
Who is collecting it

What we collect
Your email address, and your first name if you give it. If you tick "Keep my answers", we also keep your quiz answers: the areas you picked, your age range, sex and state, what is getting in the way and for how long, what you have tried, when you last had bloods, the cycle questions where they apply, the safety questions, when you would like to start, and anything you write in your own words.
Your answers are health information. Under the Privacy Act, health information is sensitive information, and we collect it only with your consent.
Why we want it
So a doctor can read your answers when bookings open, before your first consultation, and so you do not have to tell us everything twice. Your answers do not decide what tests or care you receive. A doctor decides that with you, in a live consultation.
Your consent, and changing your mind
"Keep my answers" starts unticked. Your answers are kept only if you tick it. If you leave it unticked, we keep your email only and your answers are not stored anywhere. You can withdraw your consent at any time by emailing info@indexclinic.com.au, and we will delete your answers and confirm when it is done.
If you would rather not
The quiz is optional, and so is keeping your answers. You can join the waitlist with your email alone, and you lose nothing by doing so.
Who sees it
Index Clinic staff who need it, and when bookings open, the doctor you book with. Neon stores your answers for us in Sydney. It acts on our instructions and for no other purpose. Your answers are never sent to our email platform, and never to the analytics or advertising tags on this site. We do not sell them.
Where it is stored
Your answers are stored in Australia. Your email address and first name are also held by our email platform, Klaviyo, which is based overseas, principally in the United States.
How long we keep it
Until you ask us to delete it, and no longer than we need it for the purpose above.
A copy, a correction, or a complaint
You can ask for a copy of your answers, ask us to correct them, or ask us to delete them, at info@indexclinic.com.au. If you are unhappy with how we have handled your information, tell us first. If we do not sort it out, the Office of the Australian Information Commissioner at oaic.gov.au handles it from there. Our privacy policy has the detail.
